If you are approved to take play therapy classes, this form will be signed by the Professional Counseling Program Coordinator and returned to you to include in your application to the university. Please only put ONE play therapy class on this form. Play therapy classes must be taken in the following order:

COUN 5369: Child and Adolescent Counseling Methods
COUN 5370: Intermediate Methods in Play Therapy
COUN 5346: Filial Therapy
